Fall registration is upon us and we have made a great start in getting the forms and Club fees in!  A big Thank YOU to all parents and players who have everything in. 

However, we are still missing forms and fees from a few new and returning players.  If you have not managed to make it to practice this last week because you were out of town, etc.. We would love to hear from you!

For the forms, for ALL players (new and returning), we will need the US Club Soccer, the KSYSA membership and the KS Medical form filled out and signed.  Note, the US Club Soccer form has a signature area for the player as well.  All of These forms may be found on the forms page.

For all NEW players, in addition to the forms listed above, we will need a copy of your Birth Certificate.

In addition, we will need 2 pictures of each player so we can generate the players cards for the Fall season.

Some players that have turned in their paperwork still have some missing forms, and we will be contacting you directly to bring all paperwork up to date. 

NOTE:  If we do not have all paperwork in, we cannot get players cards.  Its that simple.

Special Note:  Cindy will be out of town from 7/24 - 8/2.  Anyone planning to play in Omaha (HS and U14 Girls) needs to take care of all paperwork issues and fees by next Thursday 7/19.   Forms and fees can be turned after 7/19 to Laura Reardon (but no work on getting them processed will take place until Cindy gets back).
